Messages

You should NEVER use myCARE for emergencies. If you have an emergency call 911.

You need a current e-mail address to use myCARE.

All of the messages you send to you healthcare provider are private. However some messages may be sent to other staff members to help us get back to you more quickly.

GHC staff may not be immediately available to respond and there is no guarantee of a response time.

Please do not use any bad or abusive language in your messages. Your account may be closed if you do.

All messages sent through myCARE will be added to your medical record.

Security and Privacy

We use many security features to make sure that your information is private.

We do not share your medical information with anyone except as permitted by you or by law.

Your substitute decision-maker may access your myCARE if you are incapable of making your own decisions. You may consent to other people accessing your myCARE (ask for the Proxy Access form to give this consent).

You should not share your myCARE username or password with anyone. If you do, that person can see your records. GHC is not liable if you fail to keep your username or password confidential.

You must log in to the myCARE to see any messages we send you. No medical information will be sent to your email address.

We strongly encourage you to put a passcode on your computer or phone and install additional malware or anti-virus protection in order to ensure the complete security of your personal health information.

Neither informational websites nor myCARE can make a diagnoses or treat medical problems. This is something only your provider can do.

myCARE does not contain your complete medical record. It is only a summary of parts of your record. If you would like to see your complete record, please contact your provider, or the Health Records Office at GHC.

GHC can help with any problems you may have when using myCARE. If you have a problem you can email us at myCARE@ghc.on.ca, or call us during business hours 705-253-CARE (705-253-2273).

We try our best to make sure that myCARE is always working properly but there may be times when the system is shut down. We are not liable for any problems that may occur from being unable to see your medical information.

These Terms can be changed. When they are changed, you will be able to see the changes when logging in.

Age Restrictions:

You must be at least 16 years old to sign-up for myCARE.

Parents will be able to sign up for their children in most cases. Ask your child's healthcare provider or the GHC Health Records Office for the guidelines to access a child's myCARE.

TERMS AND CONDITIONS OF USE (Detailed)

Group Health Centre Sault Ste. Marie and District (GHC) is pleased to offer you access to your own personal health information via a web-based version of our electronic medical record system called myCARE.

DO NOT SEND MEDICAL ADVICE REQUESTS REQUIRING IMMEDIATE ATTENTION: If there is an emergency, dial 911 or visit your emergency department immediately. Do not send a medical advice request if a situation requires immediate attention. FOR ALL MEDICAL EMERGENCIES, DIAL 911.

These Terms and Conditions: If you want to set up an account to access myCARE, you must read and agree to these Terms and Conditions of Use. GHC (“we”) may change and update these Terms and Conditions of Use from time to time without notice to you.

Benefits of myCARE: You will be able to book some of your appointments online and view some of your personal health information. Also, myCARE allows you to communicate with your health care provider's office about non-emergent/non-urgent concerns or questions you may have. You understand that not all of your personal health information may appear on myCARE. If you require access to personal health information that is not on myCARE, please contact your health care provider&s office or the GHC Medical Record Department directly.

Creating Your Account: When you set up a myCARE account, you will be given a one-time only access code to activate your account. You will be asked to submit certain personal information, which will be compared against information in your patient record to confirm your identity. You may be asked to enter your health card number, your birth date, or other personal information. Once your identity is verified, you will be asked to create a user name and password that will allow you access to myCARE over the Internet. We recommend you do not use an e-mail address that you share with other people.

Protecting Your Password: myCARE will contain your personal health information. User names and passwords provide two layers of authentication and are stored in a secure database that is isolated from the Internet. As a myCARE user, your role in maintaining the security of your personal health information is: 1) Changing your password on a regular basis (you will be required to change your password every 90 days); and 2) It is your responsibility to ensure that your myCARE password is protected. Please observe the following security practices:


If you forget or lose your user name or password, contact the myCARE Administrator (see contact information at the end of these Terms and Conditions).

Protecting your computer/mobile device: We have provided a secure channel for your myCARE account. Once the information has reached your computer/mobile device, it's up to you to protect it. To protect your information, you should:


E-mail Notifications from GHC: You must provide us with an e-mail address when activating your myCARE account. It is your responsibility to keep this e-mail address current. Patients who have myCARE accounts will be notified via e-mail when there is new personal health information to be viewed on myCARE. This means that any person with access to your e-mail will be able to see this notification. Although no personal health information will be sent, the notification that new information is available by accessing myCARE may be information that you would not want others to know. You should take this into account when providing an e-mail address for notification purposes.

Response to Electronic Communication: myCARE allows you to send medical advice requests to your health care provider's office. However, GHC staff may not be immediately available to respond and there is no guarantee of a response time. Please know that if you send us a medical advice request, it may be shared with the GHC staff who assist your health care provider and with other health care providers who are covering for your health care provider during vacations and other absences. If you send a message and do not receive a response, please call your health care provider's office to follow-up.

Electronic communication is not an appropriate substitute for communication or clinical examinations, where appropriate, in-person or over the telephone, or for attending the Emergency Department when needed. You are responsible for following up on the health care provider's electronic communication and for scheduling appointments where warranted.

Web Site Links: myCARE may offer links to external medical websites. These website links are for convenience only. GHC does not endorse and has not verified the accuracy of the information on these websites. When you access an external website, you do so at your own risk. GHC is not responsible for the accuracy or reliability of this information.

Access by your SDM: In some cases a patient is not capable of making his or her own treatment decisions. In these cases, a substitute decision-maker (SDM) authorized by Ontario's Health Care Consent Act may apply for access to a patient's myCARE account through the SDM myCARE Application Form, in order to facilitate making decisions about the patient's treatment. If you are a SDM, please submit the completed SDM myCARE Application Form directly to the patient's doctor or to GHC's Medical Records Office. Upon the receipt of a SDM myCARE Application Form, GHC will verify the SDM's identity and that the SDM has the authority to make decisions on behalf of the patient. Once this information is verified, a myCARE account will be activated for the SDM allowing access to the patient's personal health information. If you are a SDM, these Terms and Conditions apply equally to your access to and use of the patient's health information. An approved SDM will be able to perform all the same functions in MyChart as a patient including adding or changing information and communicating with health care providers.

Access by Another Person with Consent: You may wish for someone else (for example, your child who supports your care ) to have access to your personal health information. If this is the case, you (the patient) should complete the myCARE Access Consent Form and submit it directly to your doctor or to GHC's Medical Records Office. Upon receipt of a myCARE Access Consent Form, GHC will verify the identity of the person you wish to have access to your myCARE information. Once this information is verified, a myCARE account will be activated for the person you have granted access, allowing that person access to and use of your personal health information. The person will have “read only” rights to the myCARE and will not be permitted to add or change information or send information to your health care providers.

Revoking Access: myCARE access is a benefit offered by GHC; it is not a right. GHC may revoke access by a patient or SDM or other authorized user to myCARE at any time. However, you or your SDM remains entitled to access your personal health information held by GHC based on the access rights given to you by Ontario's Personal Health Information Protection Act, 2004. If access to myCARE is revoked, you will still be able to access your record by contacting your health care provider directly.

Deactivating Your Account: You can discontinue use of your myCARE account at any time by contacting the myCARE Administrator (see contact information at the end of these Terms and Conditions).

GHC Information Practices: GHC affords the same degree of protection to personal health information stored on myCARE as is given to personal health information stored by GHC in any other medium, including our electronic medical record. GHC is committed to protecting the confidentiality of your personal health information. We limit our staff's access and ability to enter or view personal health information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard patient personal health information. . We are able to identify the records released and note the time and date of access each time a person accesses myCARE. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. Security: For your security, myCARE must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal. Our SSL web server uses authentication and AES encryption technology to protect information in transit.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to GHC by viewing the digital certificate. This certificate verifies the connection between the GHC server's public key and the server's identification.

This website uses encryption software. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using myCARE, information will be transmitted over a medium that is beyond our control. You expressly assume the sole risk of any unauthorized disclosure or intentional intrusion, or of any delay, failure, interruption or corruption of data or other information transmitted in connection with this service.

Limitation of Liability: GHC is not liable if you fail to keep your password secure. You can change your password at any time and we recommend you do so at least once every 3 months. GHC makes no representations or warranties of any kind in connection with MyChart.

Governing Law: By using the myCARE service, you agree that the laws of Ontario will govern these Terms and Conditions.
