If your provider is not participating, your email address will be recorded and you will be notified if they choose to go online in the future.
In order to give you access to myCARE, we will need to gather some basic information from you. Once you have submitted this form, it could take up to a week for processing and verification of your account request. Once your request has been approved, you will receive an email to the address you’ve provided with your activation code and instructions on how to activate your myCARE account.

Once your account is active, the email address you provide will only be used for myCARE notifications and feedback.
